Prolapse

Posted by editor - September 12th, 2007

Another name for prolapse is “pelvic relaxation.” Vaginal wall prolapse occurs when the pelvic muscles become weakened and allow one or several pelvic organs to drop down the vagina. Pelvic organs, including the bladder, rectum, and urethra, can prolapse into the vagina, but the most commonly affected organ is the uterus. The vaginal wall is pushed down by the unsupported and descending uterus and, depending on the severity, the cervix may protrude from the vulva.

If the rectum bulges into the back of the vaginal wall, it is called a rectocele; when the urethra bulges into the front of the vaginal wall, it is known as a urethrocele; while if the bladder drops into the front of the vaginal wall, it is called a cystocele.

Prolapse tends to occur in older women, and is much more common in those who have had children.

What Causes It?

Pelvic floor muscles can weaken with age, but prolapse is nearly always caused by earlier injury to the pelvic floor muscles, cervix, or supporting tissue of the uterus during labor, especially if you had a rapid delivery, were allowed to go on too long in labor, or if your babies were large.

Should I See The Doctor?

If your prolapse is accompanied by severe backache or pelvic discomfort, you should consult your doctor as soon as possible.

What Might The Doctor Do?

  • Your doctor will give you an internal pelvic examination to confirm a prolapse and to determine which type you have.
  • She will ask you about your deliveries for example if your babies were larger than normal, and if the second stage of labor lasted a long time.
  • Being overweight can cause the prolapse to be more troublesome, so your doctor will advise you to lose excess weight.
  • If you have a severe prolapse, your doctor may recommend surgery. This is rarely absolutely necessary, but will improve your quality of life by maintaining urinary continence and improving sexual enjoyment.
  • For mild prolapse a doctor will recommend Kegel exercises (see opposite). For older women who are too frail for surgery, there are vaginal rings to help support the vaginal walls or uterus. You may be fitted with a ring or sponge pessary that is placed high in the vagina; these, however, can erode internal tissues so should not be worn for long periods.

Surgical Treatment

  • Surgery is carried out to improve quality of life. If there are any reasons why surgery may not be a good idea, for example due to extreme frailty, it is best avoided and nonsurgical methods employed to control the symptoms. Very rarely, an anterior repair may result in urinary incontinence, and a posterior repair may sometimes result in uncomfortable or painful intercourse.
  • Most prolapse repairs are performed through the vagina and involve a general anesthetic. Occasionally the operation may be performed under an epidural anesthetic, especially if the patient is old and infirm.
  • The repair involves making an incision in the wall of the vagina and strengthening the tissues that have become weakened using buttressing sutures (stitches). Any excess or stretched tissue is removed from the vaginal walls and the incision is closed using stitches that are absorbed by the body.
  • Following the operation, the vagina is packed with gauze, which remains in place for one or two days. A catheter is inserted into the bladder during the operation to help drain urine. It is common to have some discharge from the vagina for a few days following surgery.
  • You will probably stay in the hospital for five to seven days. An outpatient appointment is usually made six weeks later, after which sexual intercourse can be resumed if there are no problems.
  • For difficult-to-treat or recurrent prolapse, an abdominal operation is occasionally carried out to strengthen the walls of the vagina.

What Can I Do?

  • If you suffer from backache, try to avoid standing for long periods at a time. Wear a tight girdle to counteract any dragging feeling you may have in your pelvis.
  • If you are having difficulty with sexual intercourse, you and your partner may need to explore alternative ways of achieving sexual pleasure.
  • Wear panty liners if you are bothered by leakage of urine (stress incontinence). If it becomes worse, see your doctor.
  • The most important preventative treatment is being conscientious about doing pelvic floor exercises regularly during pregnancy and especially after the birth of your baby, whether you have stitches or not.
